How to Add a Form to a Lesson
Step 1: "Add Form" Lesson Item
Navigate to Main Menu > Lessons and select an existing Lesson or create a new one.
Drag the "Add Form" item into your lesson.
Step 2: Configure the File Upload
Pick the Form that you have already created at Other Tools/Forms
Determine if you want to skip the form if one exists
Select whether it is required to fill out a form
